Before arranging a consultation, I ask all prospective clients to complete the questionnaire below.

This allows me to review your situation in advance and determine whether the work is appropriate and likely to be of value. It also ensures that our time together is focused and considered plans that fit their individual needs and goals.

There is no need to be exhaustive.
If something does not feel relevant, you may leave it blank.
The questionnaire typically takes 10–15 minutes to complete.

Once submitted, I will review your responses and be in contact regarding the next step.

Not all enquiries proceed to consultation. If I feel another form of care would be more suitable, I will advise you accordingly.
